• Medical Requirement: Include a letter from a licensed physician confirming diagnosis of heart disease or cardiac deformity (medical records are not accepted).
• Recommendation: One letter from a current or former professor supporting the applicant’s character and/or academic promise.
• Personal Statement: A 500–2,000 word essay describing obstacles related to the cardiac condition and how they’ve been overcome.
